Case Coordination Services

Case coordination is a separate program within the agency for accepting and appraising referrals for service. It is responsible for assessing the service needs of referrals and assisting the individual and families in obtaining or linking service needs.

Services Include:

• Assessment of Service Need
• Development of Individual Service Plan
• Assist in the arrangement of service delivery
• Coordination and advocacy with Service Providers
• Follow-up monitoring services
• Supportive counseling
